CHECK LIST

1) Complete a Wilder Award Entry Form for each entry and affix a copy of the form to the back of each entry submitted. (Do not attach with paper clip)

2) Please only one entry per category.

3) All entries must be accompanied by a digital image of the entry. Submit a CD-Rom with one photograph per entry (image size: 1024 pixels x 768 pixels at 72dpi).

4) Complete a Master Registration Form and submit with your entry/entries.
